To compare the accuracy of intraocular lens (IOL) power prediction using Haigis, SRK II & SRK T formulae in eyes of different axial length (AXL) undergoing phacoemulsification

Abstract Details

Purpose:

To predict accuracy of different IOL formulae in eyes with different AXL, to determine the most appropriate IOL formula in eyes with different AXL & to compare Haigis & Hoffer Q formulae in eyes with short AXL.

Setting:

Hospital based interventional study with duration of 18 months including patients undergoing clear corneal temporal phacoemulsification. Alakh Nayan Mandir, Institute of Ophthalmology, Udaipur,India

Methods:

Age group was 40-75 years. 33 eyes with immature senile catarac were divided in 3 groups; 60 eyes with AXL between 18.21.99 mm(Group I), 180 eyes with AXL between 22-24.49 mm(Group II) & 60 eyes with AXL between 24.50-30 mm(Group III).A-scan was done by immersion technique. IOL was implanted by Haigis, SRK II & SRK T formulae in group I, II & III respectively. Postoperative refraction was done at one month, spherical equivalent (SE) calculated & estimated by other formula in each group. Mean SE was calculated for each formula. Paired-t test & p-value were used for analysis.

Results:

Mean SE with Haigis was -0.45+/-0.73D & predicted mean SE with SRK II, SRK T & Hoffer Q were +0.70+/-0.85D, +0.59+/-0.81D & +0.44+/-0.74D respectively in group I. 75% eyes (45 eyes) were within +/-1.00D with 35 myopic or emmetropic & 10 hypermetropic with Haigis, while 70% eyes (42 eyes) were within +/- 1.00D with 17 myopic or emmetropic & 25 hypermetropic with Hoffer Q. Mean SE with SRK II was -0.04+/-0.48D & predicted mean SE with SRK T & Haigis were -0.11+/-0.49D & -0.55+/-0.67D respectively in group II. Mean SE with SRK T was -0.32+/-0.48D & predicted mean SE with SRK II & Haigis were -0.68+/-0 73D & -0.51+/-0.66D respectively in group III.

Conclusions:

Haigis formula works well for eyes with AXL between 18-21.99 mm followed by Hoffer Q, while SRK II & SRK T gave results towards hypermetropic side. 75% & 70% eyes were within +/-1.00D with Haigis & Hoffer Q respectively. SRK II formula works well for eyes with AXL between 22-24.49 mm followed by SRK T while Haigis gave results towards myopic side. SRK T formula works well for eyes with AXL between 24.50-30 mm followed by Haggis while SRK II gave results towards myopic side.
