Abstract Details

Purpose:

To determine the accuracy of intraocular lens (IOL) calculous with third generation formulas and so to establish the best final refractive outcome.

Setting:

Miguel Servet University Hospital, Zaragoza, Spain.

Methods:

Six hundred sixty five eyes with implantation of Tecnis monofocal ZCB00 IOL were examined. The IOL power was calculated with optical biometry, IOL Master. One month after the surgery, the refractive defect and the obtained spherical equivalent were measured and so the difference with the expected spherical equivalent was calculated with the following formulas: SRK/T, Haigis, Holladay I and Hoffer-Q. It was analysed if there were any statistically significant differences between the used formulas or between the axial lengths (AL) of the eyes.

Results:

Fifty five short eyes whose AL was 21.52 ± 0.38 mm were evaluated. The most accurate formula was Haigis. There were differences between all the formulas except between SRK/T and Holladay I. Fifty four long eyes whose AL was 26.37 ± 1.26 mm were evaluated. The most accurate formula was SRK/T. There were differences between Haigis and Holladay I, and Haigis and Hoffer-Q. Five hundred fifty six medium eyes whose AL was 23.47 ± 1.33 mm were evaluated. The most accurate formula was Haigis. There were differences between SRK/T and Holladay I, SRK/T and Hoffer-Q, and Holladay I and Hoffer-Q.

Conclusions:

The election of an adequate formula for Tecnis ZCB00 IOL power calculous has to take into account the AL because the calculous precision depends on it. For most eyes (medium AL) SRK/T formula could be used, whereas Haigis formula is more adequate for eyes with extreme AL.
