Comparison of IOL power calculation formulas using Lenstar in patients with different axial lengths

Abstract Details
Purpose:
To evaluate and compare accuracy of different intraocular lens (IOL) power calculation formulas using Lenstar in eyes with various axial length (AXL).
Setting:
Bozok University Faculty of Medicine, Ophthalmology Department, Yozgat, Turkey.
Methods:
165 eyes of 165 patients having uncomplicated phacoemulsification and IOL implantation were included to this prospective study. Biometric measurements were performed with the optical low-coherence reflectometry (Lenstar). The patients were divided into short (<22 mm), average (22-25 mm) and long (>25 mm) AXL groups according to their preoperative AXL measurements. Postoperative refraction results compared with the predicted refractions calculated using Olsen, SRK/T, Holladay II, Hoffer Q and Haigis for each AXL group. The mean and median absolute errors (MAE, MedAE) and percentage of eyes within, ±0.50 D of predicted refraction and deviating over ±1.00 D of predicted refraction were calculated.
Results:
In average AXL group, there was no statistically significant difference in MAE and MedAE among the formulas. The MAE was 0.34±0.37 D in Haigis, 0.38±0.36 D in Olsen, 0.35±0.32 D in Holladay II and 0.37±0.39 D in SRK/T, respectively. In the long AXL group, the Haigis achieved the lowest value of MAE (0.32±0.36 D) followed by the SRK-T (p<0.05 for Haigis versus Hoffer-Q). In the short AXL group, the percentage of eyes within ±0.50 D of predicted refraction was higher with the Haigis (77%) than the other formulas including the Hoffer-Q (64%).
Conclusions:
When choosing the IOL power, selecting the appropriate formulas would optimize the refractive results.
