Phacoemusification, how to choose the right intraocular lens and why?

Purpose:
Choosing the right intraocular lens after phacoemulsification is real challenge.
Setting:
Alcon infinity Phacoemulsification System
Methods:
In the period from June 2010 until February 2016, in our hospital were performed 4247 Phacoemulsification procedures with implantation of AcrySof® intraocular lenses. Patients aged from 18 months to 92 years old, 2158 of them women and 2089 men. All the patients underwent complete preoperative examination with standard protocol: visual acuity, biomcroscopic examination, intraocular pressure examination, fundus examination , biometry, corneal topography (Pentacam), OCT and IOL toric calculator.
All the patients were operated with standard phacoemulsification protocol and were implanted AcrySof® intraocular lenses. We used monofocal, muliti piece and one piece (usually yellow), toric, multifocal and multifocal toric lenses.
Results:
Postoperative follow up in the first postoperative day, after a week, after a month and then in the interval of two months until 1 year period. Visual acuity measured 0.8 -1.0 in 3695 patients (87%) without any pathological change in the posterior segment of the eye, 0.3 to 0.8 in 350 patients with associated posterior segment changes (optic nerve atrophy, macular degeneration, diabetic macular oedema) and lower visual acuity measured from 0.1 to 0.5 in 202 patients with high refractive errors and amblyopia.
Conclusions:
Choosing the right intraocular lens with aspheric design, biocompatibility, yellow protective filter, lower PCO incidence, precise and consistent IOL alignment in the capsular bag are crucial for fast and efficient postoperative visual acuity achievement. Choosing toric and multifocal lens offers complete glasses liberation.
