Purpose:
We want to help surgeons understanding the outcome of the surgery on a objective basis and not only on a subjective opinion made on patient satisfaction. the main problem encountered in the post op. care of such patients is to familiarize with the results based on autorefraction/wawefront analysis and visual acuity testing, knowing witch outcome in the first post op.days will have a good result for far and near vision.

Methods:
We tested all the cases with (OPTICAL PATH DIFFERENCE SCANNING SYSTEM) OPD and revise data of 70 implanted eyes in the last three years, with a new IOL’s family, made on a asymmetrical optic design defined paraxial asphericity with an extended depth of focus function. We divided the patients in two groups, implanted with high add and low add intraocular lenses. We made a retrospective analysis of the wavefront patterns obtained from implanted eyes correlating it with the clinical and subjective results.
Results:
We noted especially in the high add IOL that the internal wavefront pattern was aligned with the optical axis at the limit of the additional near vision paraxial segment. All eyes, were the lens was pretty good centered on the pupil and the visual axis, we found an auto-refraction value between -0,75 and -1,25, this false myopia is not requested by the objective testing of the visual acuity, and patients have good visual acuity for far and near. With the OPD pattern we demonstrated in two cases a mild decentration of the IOL with reduction in the near vision ability.
Conclusions:
New multifocal IOL subjective and objective results are affected by lens position in the antero posterior axis and in pupil centration. Wavefront analysis of the total and internal OPD is mandatory to understand the real IOL pattern and the correlation between cornea and pupil . In some cases nureroadapatation, pupil size and corneal astigmatism can affect the visual performance for the first period and knowing exactly what is going on may help surgeon in a decision making process.
