Comparison of balanced 30-degree and 45-degree tips of Centurion vision system in cataract extraction surgery

Abstract Details

Purpose:

To compare the outcome measurements with balanced 30-degree and 45-degree tips obtained from Centurion Vision System during phacoemulsification.

Setting:

Oftalvist CIO Jerez, Jerez de la Frontera, Spain

Methods:

A prospective study was carried out in patients undergoing cataract surgery with the phacoemulsification device Centurion Vision System using either a 30-degree or a 45-degree Intrepid Balanced phaco tips. Intraoperative measurements were collected at chop#1, chop#2 and final settings including total case time, ultrasound (US) total time, average torsional and longitudinal amplitudes, cumulative dissipated energy (CDE), and balanced salt solution (BSS) use. The endothelial cell density was also evaluated preoperatively and postoperatively.

Results:

There were differences for total aspiration time and total case time only at chop#2 setting between two tips (p<0.05). Average longitudinal power, average longitudinal power FP3, total longitudinal power on time, average torsional amplitude, average torsional amplitude FP3 and equivalent average ultrasonic power FP3 were significant different between two tips at all settings measured. The mean CDE was increased in balanced 45-degree tip at chop#1, chop#2 and final settings (p<0.05, p<0.01 and p<0.01, respectively). There were no significant differences in total aspiration time and endothelial cell loss between tips.

Conclusions:

Phacoemulsification performed with balanced 30-degree and 45-degree tips showed significant differences in Centurion CDE measurements, however balanced 45-degree tip was effective in lens removal with low CDE values. Balanced 45-degree is safer for phacoemulsification of hard cataracts being an alternative phaco tip for many surgeons' preference.
