Abstract Details
Purpose:
To demonstrate feasibility, safety and efficacy of Vacuum assisted capsulorrhexis during phaco-emulsification without using ocular viscoelastic devices (OVD).
Setting:
Anjani Eye Hospital, Nagpur, India
Methods:
Retrospective analysis of 8000 consecutive cases of cataract surgeries performed by phacoemulsification with foldable IOL implantation by a single surgeon from 2005 to 2015. An anterior chamber maintainer (ACM) was introduced at infero-temporal quadrant through a side-port entry at the start of the surgery & connected separately to the BSS infusion bottle. Capsulorrhexis was initiated with a 26G bent needle. The free flap of the capsule was then grasped with a hydrodissection canal mounted on a 5cc syringe with a Luer lock. The flap was held at various locations so as to achieve a complete rhexis.
Results:
98% of the cases did not need an OVD during the rhexis. Few cases with hyper mature cataracts or those with pre-existing zonular dialysis had to have injection of OVD intraocularly and the rhexis completed.
Conclusions:
Our results suggest that a safe and continuous capsulorrhexis (CCC) can be performed during phacoemulsification with the use of vacuum created in a cannula.. It does need a gradual learning curve which can be overcome..
