Three ways of non-conventional use of iris retractors

Abstract Details

Purpose:

iris retractors are used for pupil enlargement in cases of inadequate mydriasis. However, commorbidity, such as general (due to pseudoexfoliation) or local (i.e. traumatic) poor zonular stability or zonular rupture (iatrogenic) may occur and necessitate further capsular support. The purpose of the study is to present ways of using already placed (1 case) or readily available (two cases) iris retractors to achieve stabilization of the capsule.

Setting:

Department of Ophthalmology, General Hospital of Lamia, Lamia, Greece

Methods:

1st case: a case of mild phacodonesis in an eye with pseudoexfoliation syndrome where the already placed iris retractors were used to stabilize the capsule via stabilization of the anterior capsulorrhexis. 2nd case : in an eye with traumatic partial zonular rupture iris retractors were used to prevent lens tilting prior to insertion of capsular tension ring. 3rd case: in a iatrogenic partial zonular rupture during posterior chamber intraocular lens( IOL PC) insertion iris retractors were used to expand the capsular bag allowing for proper IOL PC implantation.

Results:

In all three cases (zonular instability, traumatic and iatrogenic zonular rupture) iris retractors served well in stabilizing the capsule during the intraoperative manipulations.

Conclusions:

conventional surgical tools in non-conventinal roles may allow for greater intraoperative flexibility in unexpected situations.
