Changes in corneal incision width after phacoemulsification and after foldable lenS implantation: comparison between different delivery systems

Purpose:
to compare corneal incision sizes after phacoemulsification and after intraocular lenses implantation (IOL) using different delivery systems.
Setting:
Department of Surgery and Biomedical Science, Ophthalmology Section, S.M. della Misericordia Hospital, University of Perugia, Italy
Methods:
One hundred and seventeen patients with cataract and without other anterior segment pathological features or previous eye surgery were enrolled. We performed phaco-chop technique with IOL implantation through a 2.2 mm incision. Three foldable lenses were used with their recommended delivery systems: Alcon SN60WF with Monarch III/cartridge D (Group A, 38 patients); Amo Tecnis ZCB00 with Unfolder Platinum/cartridge easy load (Group B, 38 patients); Alcon AcrySoft IQ with Ultrasert preloaded system (Group C, 42 patients). We measured incision width before and after phacoemulsification as well as before and after IOL implantation.
Results:
Incision width before and after phacoemulsification was 2,21±0.02 mm and 2,34±0.08 mm in group A; 2,20 ± 0.02 mm and 2,31 ± 0.06 mm in group B; 2,20 ± 0.02 mm and 2,30 ± 0.07 mm in group C. Incision width did not show statistically significant enlargement after phacoemulsification. The incision width before and after IOL implantation was 2,34 ± 0.07 mm and 2,47 ± 0.07 mm in group A; 2,32 ± 0.06 mm and 2,45 ± 0.08 mm in group B; 2,30 ± 0.07 mm and 2,39 ± 0.07 mm in group C. The difference between values in group C and in other groups was statistically significant. No relationship between incision sizes and phacoemulsification time, ultrasonic energy, IOL powers.
Conclusions:
The use of Alcon AcrySoft IQ IOL with Ultrasert preloaded system induces lower corneal incision enlargement in cataract surgery compared to other delivery systems.
