Purpose:
We present a new endocapsular ring for cataract surgery to reduce or avoid PCO, IOL dislocation and to facilitate explantation or secondary implant of an intraocular lensA totally new concept of capsular ring for cataract surgery is here introduced and can be reconducted to surgical devices and to intraocular prosthetic devices already in use such as intraocular lens and other varieties of capsular tension rings.

Methods:
To describe a new continuous capsular ring renovated in its three-dimensional characteristics. 1) The design of the internal “U” section, thus permitting the placement of the haptics of intraocular lenses. 2) The sharp square external borders, creating a barrier against the opacification of the posterior capsule (PCO). 3) The thickness of the ring preventing contact between the anterior and the posterior capsule, maintains an open bag. 4) The implant of the ring can be carried out with the same standard injectors already in use for intraocular foldable lenses, by using1,8 - 2mm incision size.
Results:
101 Eye have been implanted in the last four years with this ‘equatorial capsular ring’ without adverse reaction or side effect attributable to the device itself.
Conclusions:
Better intraocular lens centration on x y z axis and less or none PCO after 5 years from implantation are the benefits of this new bag ring, aiming to a new concept, an open bag could be the answer to eliminate, reduce or stop physiologically the PCO.
