Impact of selective alpha 1A receptor antagonists on pupil dilation after instillation of tropicamide 0.5% and phenylephrine 10% and its correlation with the degree of intraoperative floppy iris syndrome (IFIS)

Abstract Details

Purpose:

The purpose of this study was to evaluate the impact of the intake of selective alpha 1A receptor antagonists on pupil dilation after instillation of tropicamide 0.5% and phenylephrine 10% and its correlation with IFIS which was graded concerning initial and subsequent pupil size as well as by measuring the absolute and the relative iris deflection by the infusion.

Setting:

Vienna Institute for Research in Ocular Surgery, Department of Ophthalmology, Hanusch Hospital, Vienna

Methods:

Patients who were scheduled for cataract surgery and who either took a selective alpha 1A receptor antagonist perorally (study group) or who had no history of an intake of alpha receptor antagonists (control group) were included in this study. The thickness of their pupil dilator muscle region was measured preoperatively using SD-OCT. Their pupil size was measured preoperatively before and after mydriasis as well as intraoperatively before incision, after phacoemulsification, after irrigation/aspiration and after IOL implantation. Furthermore the degree of IFIS as well as the frequency of intraoperative complications such as phaco bites and iris prolapses was assessed.

Results:

Twenty-five eyes of 25 patients were included per group. This study only included male patients because of the main indication “benign prostate hyperplasia” of alpha 1A antagonists intake. One eye of every patient was included. Mean age of patients in the study group and in the control group was 77±4 years and 74±6 years, respectively (p=0.55). Mean duration of the alpha 1A receptor antagonist intake in the study group was 6.5±3.7 years. Detailed results will be presented.

Conclusions:

The extent of IFIS varies greatly among patients on alpha 1A receptor antagonists. The success of using OCT of the iris muscles as well as pupil dilation before surgery as predictors of IFIS will be discussed.
