Descemet detachment in femtosecond laser-assisted cataract surgery and the isolate-and-release technique

Abstract Details
Purpose:
To report cases of Descemet detachment in femtosecond laser-assisted cataract surgery (FLACS) and the isolate-and-release technique to manage it.
Setting:
HOPE Eye Hospital, in Recife, Brazil.
Methods:
In this retrospective study, the charts of patients that had undergone FLACS between September 2014 and September 2015 were assessed. All patients that had Descemet detachment during the femtosecond laser application were identified and their cases reviewed.
Results:
Ten eyes had localized Descemet detachment during FLACS. One patient had mild guttae and bilateral detachment. Routine laser parameters were used and the laser presented no abnormality in calibration tests. All detachments were successfully managed using the isolate-and-release technique, which consists of first opening the incision not affected by the detachment and filling the anterior chamber with ophthalmic viscosurgical device. Then, the incision with the detachment is opened, resulting in gas bubbles leaving the eye and the detachment’s limits becoming only barely seen. Optical coherence tomography revealed a localized detachment in the first postoperative day, which resolved within 30 days.
Conclusions:
Descemet detachment, a possible complication of FLACS, can be effectively managed with the isolate-and-release technique.
