The analysis of effectiveness of therapeutic treatment of posterior capsule opacification

Abstract Details

Purpose:

At present, there are no reliable effective methods of therapeutic influence on proliferated lens epithelial cells (LECs). Thus, the purpose of our work was to perform the clinical analysis of presence of cytotoxic impact of osmotic medications on LECs at posterior capsule opacification (PCO).

Setting:

State Institution Eye Microsurgery Complex named after academician S.N. Fyodorov, Khabarovsk, Russia

Methods:

The observation group consisted of 28 patients (28 pseudophakic eyes) with PCO, where LECs hyperplasia (Elshnig pearls) was the main component of opacification. All patients previously underwent uneventful PE with IOL implantation because of senile cataract. During 1-month period, all patients used 3 times per day instillations of NaCl 5% solution. UCVA and BCVA were examined before treatment, 1 and 3 months after the end of treatment. Biomicroscopy and obligatory photo registration of posterior capsule (PC) status were performed at the same terms.

Results:

The initial indexes of UCVA varied from 0.05 to 0.3, BCVA made from 0.2 to 0.6 (average 0.18±0.1 and 0.34±0.13, correspondingly). One month later the mean values of UCVA made 0.25±0.11, of BCVA – 0.42±0.12 . The elevation of both values in the group was determined by decrease of PCO sizes and intensity in 6 eyes (24.4%). 3 month after the end of treatment, mean UCVA was 0.17±0.12 and mean BCVA was 0.32±0.14 in 27 eyes, which met the initial characteristics and was caused by regress of achieved effect in 4 eyes. There were no LECs proliferations in two eyes.

Conclusions:

The performed analysis showed the presence of cytotoxic impact of osmotic medicines on LECs at PCO. In spite of low influence rate and instability of therapeutic effect of NaCl 5% solution, this direction (in our opinion) is rather promising and worth further investigation.
