IOL power prediction after myopic refractive laser surgery: a retrospective analysis and prospective validation study first results

Abstract Details

Purpose:

To investigate refractive outcome in a prospective patient group, using the results of a former retrospective analysis on methods of IOL power prediction after myopic refractive laser surgery.

Setting:

Prospective investigation of post myopic refractive laser patients in the Utrecht University Medical Center.

Methods:

In 2012 we performed a retrospective analysis on IOL power prediction methods in a cohort of 70 eyes that had underwent refractive laser surgery. At that time no special calculation software was available. We investigated different corrective methods for K-values and different formulas and tools. From this cohort the Refractive Derived Corrected K-values, applied in the Holladay and SRK/T, the equivalent K-readings in the Haigis formula, and the Masket formula of the ASCRS tool showed the best predictive values. From 2014 we applied these methods on patients in the Utrecht University Medical Center

Results:

From July 2014 until March 2016, 10 post refractive laser surgery eyes underwent cataract extraction followed by placing an intraocular lens. 7 eyes were treated for myopia, 3 for hypermetropia. For all 7 formerly myopic eyes, the 4 calculation methods noted in the methods were used for prediction. 4 eyes had a postoperative refractive error of less than 0.5 Diopters from target refraction. Of 3 eyes, no postoperative refractive error is available yet, due to short follow-up time after recent surgery (n=2) and postoperative CME (n=1) in a uveitis patient.

Conclusions:

Both populations of the Rotterdam Eye Hospital and the Utrecht University Medical Center contain a wide variation of patient characteristics and ocular pathology. No patients were excluded from both studies, since these are the actual patients to treat. Although a small number of eyes has been investigated yet, Refractive Derived Corrected K-values applied in the Holladay and SRK/T formulas, Equivalent K-readings applied in the Haigis formula and the Masket formula of the online ASCRS tool seam to be accurate in predicting IOL power in patients with former myopic refractive laser surgery.
