Comparative analysis of optical quality in unused versus opacified and explanted hydrophilic intraocular lenses

Abstract Details

Purpose:

Optical bench analysis and comparison of opacified and explanted versus new and clear Oculentis Lentis-LS-502-1 single piece hydrophilic acrylic blue light filtering intraocular lens (IOL).

Setting:

International Vision Correction Research Centre (IVCRC) & David J Apple International Laboratory for Ophthalmic Pathology ,Dept. of Ophthalmology, Univ. of Heidelberg, Germany

Methods:

In a laboratory study we compared the optical quality of 2 opacified and 2 clear Oculentis Lentis-LS-502-1 IOLs (+19,5 and +22,5 D) using the OptiSpheric IOL (Trioptics, Germany) optical bench. We measured modulation transfer function (MTF) in sagittal (MTFsag) and tangential (MTFtan) planes, at 50 lp/mm and 100 lp/mm. MTF area (MTFA) values are also calculated and measurements of relative light transmission are performed. Furthermore we compared visually the quality of images produced by these IOLs using USAF ( United States Air Force ) target analysis.

Results:

For the +19,5 D clear IOL, MTF @50 lp/mm values were sagital mean: 0,772 (SD: 0,003) and tangential mean: 0,783 (SD: 0,003). These values were for the opacified, explanted +19,5D IOL 0,760 (SD: 0,003) and 0,778 (SD: 0,001) respectively. For the +22,5 D clear IOL, MTF @50 lp/mm values were sagital mean: 0,756 (SD: 0,005) and tangential mean: 0,753 (SD: 0,004). And for the opacified, explanted +22,5 D IOL 0,736 (SD: 0,008) and 0,686 (SD: 0,001) respectively. The relative transmission values were for the opacified +19,5 D IOL 56,52% and for the +22,5 D IOL 57,38%. The image quality of the USAF target was better with the clear IOL compared to that with the explanted, opacified one.

Conclusions:

MTF and MTFA values were better with the clear IOLs. The image quality was better with the clear IOLs as confirmed by USAF target pictures. Lower MTF and relative transmission values correlate with the reduced visual acuity of the patients after the opacification of the IOL. The higher difference between sagital and tangential MTF values of the opacified IOL show an overall deterioration of the optical quality which may be caused by the structural changes in the optic due to manipulations during the explantation or the inhomogeneous opacifications in the optic.
