Symptomatic vitreo-macular adhesion as pathogenetic factor in the development of macular edema after phacoemulsification

Abstract Details

Purpose:

The describe clinical cases of macular edema in patients undergoing phacoemulsification, and to evaluate the role of symptomatic vitreo-macular adhesion in its development.

Setting:

Ophthalmological Clinic 'Eye Surgery' Samara,Russia

Methods:

Presented two clinical cases, where, after standard phacoemulsification and implantation of an intraocular lens developed cystoid macular edema. Examination, including OCT, revealed the presence of symptomatic vitreo-macular adhesion. Monitored over the treatment and status of vitreo-macular interface with OCT weekly-control; special attention is paid to the dynamics of symptomatic vitreo-macular adhesion.

Results:

As a result of conservative treatment achieved good functional and anatomical results – complete detachment of the posterior hyaloid membrane and the elimination thereby of symptomatic vitreo-macular adhesion. Discusses the role of symptomatic vitreo-macular adhesion and its traction component, the critical increase during the phacoemulsification led to the formation of macular edema.

Conclusions:

In the pathogenesis of postoperative macular edema significant role is played by the original symptomatic vitreo-macular adhesion. Marked fluctuations of the vitreous body due to fluctuations of intraocular pressure during phacoemulsification reinforce the traction component of the symptomatic vitreo-macular adhesion and can contribute to the development of macular edema. Timely conservative treatment may lead to complete detachment of the posterior hyaloid membrane and the elimination of symptomatic vitreo-macular adhesion with positive anatomical and functional results.
