Bilateral luxation of the intraocular lens: capsular bag complex in a patient with pigmentary retinopathy

Abstract Details
Purpose:
To report an unusual case of bilateral luxation of the intraocular lens (IOL) – capsular bag complex in a 57-year-old woman with pigmentary retinopathy (PR), after a fall.
Setting:
Department of Ophthalmology, Centro Hospitalar de Leiria, Leiria, Portugal.
Methods:
Clinical case report of a 57-year-old woman with PR, who was referred to our department due to decreased vision after a fall without direct eye trauma. She underwent phacoemulsification in both eyes with in-the-bag single piece acrylic foldable IOL implantation eight years back.
Results:
The best corrected visual acuity (BCVA) was counting fingers at 50 cm in the right eye (RE) and light perception in the left eye (LE). Ophthalmologic examination showed iridodonesis and a complete luxation of IOL - capsular bag complex bilaterally. The IOL - capsular bag complex was explanted and exchanged with an iris-fixated anterior chamber polymethylmethacrylate IOL through a limbal incision, combined with a pars plana 27-gauge anterior vitrectomy. Vision improved in both eyes following surgery, without any associated ocular morbidity.
Conclusions:
The luxation of the IOL – capsular bag complex, sometimes bilateral, is a late complication of cataract surgery that can be managed by IOLs exchange. The replacement with an anterior chamber IOL is a safe and effective treatment option, held in the present case. We believe that zonular weakness secondary to PR was the predisposing factor in this case.
