A rare case of bilateral postoperative capsular block syndrome with unusually delayed onset

Abstract Details

Purpose:

To present a rare case of a patient with a bilateral late postoperative capsular block syndrome, following uncomplicated cataract surgery and discuss the treatment for this type of complication.

Setting:

Ophthalmology Department, St. Joseph Stift Krankenhaus, Bremen, Germany

Methods:

An 81- year old male patient presented in our clinic with a gradual visual loss in both eyes. He had a history of an uncomplicated cataract surgery 8 years earlier. Ophthalmic examination revealed a best corrected visual acuity of 20/ 32 in the right eye and 20/40 in the left eye. A collection of milky turbid fluid was noted in the retro-lenticular space between the intraocular lens and the posterior capsule in both eyes. The patient underwent a Nd:YAG laser posterior capsulotomy on his right eye. The left eye is scheduled for April 2016.

Results:

The patient reported a rapid visual recovery. 10 days after the treatment the best corrected visual acuity on his right eye was 20/22. There were no signs of inflammation or other postlaser complications.

Conclusions:

The late postoperative capsular block syndrome is a rare complication of phacoemulsification surgery. It can present after a long time and cause a significant visual impairment. Our case presents a very rare occurrence with a bilateral presentation. Nd:YAG laser posterior capsulotomy was a safe and effective method of treatment.
