Phacoemulsification of complicated cataract affected by silicone oil tamponade of vitreous cavity

Abstract Details

Purpose:

To evaluate the efficacy of surgical technique of phacoemulsification of complicated cataract affected by silicone oil tamponade of the vitreous cavity.

Setting:

The S. Fyodorov Eye Microsurgery Federal State Institution Orenburg Branch, Orenburg, Russia

Methods:

The surgeon faces the problem of phacoemulsification of complicated cataract in eyes with silicone oil tamponade. Paracentesis are performed at 2 and 9 hours, corneal incision at 11 hours. Viscoelastic of high specific gravity is introduced into the anterior chamber. 5 mm capsulorhexis is performed by collet forceps followed by standard phacoemulsification procedure with phaco-chop technique. To keep camera stable and prevent prolapse of the posterior capsule when removing the last nucleus pieces a heavy viscoelastic is introduced into the posterior chamber. Intracapsular ring is implanted before irrigation-aspiration stage, thus preserving stability of the lens capsule. Then IOL is implanted.

Results:

30 patients have been operated according to this method. 2 cases had posterior capsule rupture during the surgery. In these cases, IOL was implanted into the capsule and fixed with polypropylene suture 10/0 to the iris. In the postoperative period the position of the IOL was right and stable in all cases during the entire period of observation. Long-term results were followed up to one year.

Conclusions:

The analysis of this approach to complicated cataract surgery affected by silicone oil tamponade of vitreous cavity has shown its efficacy. Its peculiarity is the application of viscoelastic materials of high specific gravity during the surgery.
