Refractive outcomes after sulcus IOL implantation

Purpose:
The aim of the study was to evaluate the refractive result in patients with sulcus IOL implantation due to lens capsule rupture.
Setting:
2nd Ophthalmology Department, Aristotle University of Thessaloniki, Thessaloniki, Greece
Methods:
The records of patients underwent cataract surgery from January 2014 to December 2015 were reviewed. Eyes in this study underwent phacoemulsification and IOL implantation in the ciliary sulcus because of capsule rupture, with topical anaesthesia. Patients in our study population had their IOL power reduced by 0.5 or 1 diopter (D) from that calculated for in-the-bag implantation. The IOL implanted in all cases was the foldable 3-piece acrylic Acrysof MA60AC. For all eligible patients, the difference between actual spherical equivalent (SE) refraction and that predicted by biometry was calculated. In addition, we investigated the presence of any IOL induced astigmatism.
Results:
Our results confirmed that the final outcome is dependent on factors as eye’s axial length and IOL power. Lens-induced astigmatism is negligible in the vast majority of cases, except from cases with significant tilting.
Conclusions:
There is a need for meticulous preparation of the surgeon for appropriate preparation for the event of sulcus implantation due to capsule rupture or significant zonular weakness. Relevant nomograms should be available in all operating rooms.
