Endothelial cell loss after phacoemulsification: comparison between the Centurion® vision system’s active fluidics™ and the gravity fluidics technology

Purpose:
The aim of the study was to investigate whether the Centurion® Vision System’s Active Fluidics™ technology might contribute to less corneal endothelial cell loss compared to the gravity fluidics technology after uneventful phacoemulsification.
Setting:
This prospective study was conducted in June and July 2015 at the operative unit of ophthalmology, hospital of Chiari
Methods:
Thirty eyes of 30 patients were randomly assigned to two groups: 15 patients (group A) underwent cataract surgery using the Centurion® Vision System’s Active Fluidics™ technology and 15 patients (group B) using the gravity fluidics technology. Corneal endothelial cell counts were done in the central part of the cornea using a non-contact automatic microscope preoperatively, 7 and 30 days after surgery. A single examiner performed all the measurements. All operations were done by the same surgeon. Statistical analysis was performed by using Student’s t test. A p value less than 0.05 was considered statistically significant.
Results:
Preoperative mean cell density (MCD) was 2437 cells/mm² in group A and 2085 in group B (p=0,07). In group A MCD was 2175 after 7 days (p=0,09), and 2004 after 30 days (p=0,04). In group B MCD was 1554 after 7 days (p=0,02) and 1510 after 30 days (p=0,01). The percentage of cell loss in group A was 11% after 7 days, and 18% after 30 days; in group B it was 26% after 7 days and 28% after 30 days.
Conclusions:
The results of the study suggest that the Centurion® Vision System’s Active Fluidics™ technology might lead to a corneal endothelial cells’ saving compared to the gravity technology at least for the first 7 days after surgery. This preliminary finding is interesting, although the number of patients is small: therefore, larger studies are necessary to confirm this result.
