Bean-shaped ring capsule enhancement in a child presenting ectopia lentis et pupillae syndrome

Abstract Details

Purpose:

To describe the surgical management of ectopia lentis et pupillae syndrome in a 7-year old female child

Setting:

Dept of Ophthalmology, University Hospital Antwerp, Antwerp, Belgium.

Methods:

Interventional case report of novel intra-ocular lens (IOL) fixation in a rare autosomal recessive condition.

Results:

A 7-year old girl was referred with clinical signs of ectopia lentis et pupillae syndrome. Slitlamp examination revealed an inferior pupillary displacement and a lens subluxation in the opposite direction. Suspensory zonular loss was marked inferiorly, but normal superiorly. The bag-in-the-lens intra-ocular lens technique was used to accommodate this small ill-suspended capsule and to prevent secondary cataract formation in this young child. To offer additional capsular support, a capsular tension ring and 2 calculated bean-shaped segments were inserted. The use of beans was to provide an additional capsular support over the area of defect and to optimally centre the capsule-IOL complex

Conclusions:

Ectopia lentis is a rare disease and its surgical management remains a challenge because of the lack of zonular support. Bean-shaped capsular implants provide a scaffold for the insertion and centration of a bag-in-the-lens IOL in this rare disorder.
