Abstract Details

Purpose:

To compare the refractive and visual outcomes collected during the first six postoperative months after the implantation of two types of different one-piece acrylic diffractive multifocal intraocular lenses (IOL).

Setting:

Semmelweis University, Department of Ophthalmology, Budapest, Hungary

Methods:

Cataract surgery was performed on 100 eyes of 50 patients using phacoemulsification technique. Medicontur Bi-Flex 677MY IOL was implanted in 50 eyes of 25 patients (Group A). Alcon Acrysof Restor SN6AD1 IOL was implanted into 50 eyes of 25 other patients (Group B). Postoperative examinations were performed 1 and 7 days, 1, 3 and 6 months after the surgery. Clinical outcomes were collected including uncorrected (UVA) and corrected LogMAR visual acuity (CVA) for 4 meters, 80 and 40 centimeters, measured and manifest refraction. Depth of focus was characterized using defocus curve. Patients’ satisfaction was tested with visual functioning questionnaire (VFQ).

Results:

Uncorrected and corrected visual acuity significantly increased in both study groups, for all three analysed distance (3-month postop. unilateral UDVA (A): 0,06±0,09; (B): 0,06±0,09; UIVA (A): 0,22±0,15; (B): 0,30±0,11; UNVA (A): 0,16±0,08; (B): 0,19±0,09). Measured and manifest refraction showed a significant reduction similarly in both cohorts (Wilcoxon; p<0.05). Visual outcomes were also found to be similar in the two groups, though UIVA in Group A was significantly better at 3 months (Mann-Whitney, p<0.05). Significant (17.4%) difference was found between the area under the defocus curves (Group A: 5,94 D; Group B: 5,06 D; Mann-Whitney, p<0,001). Evaluation of VFQs resulted similarly good outcomes in both groups.

Conclusions:

Multifocal IOL implantation is a well-calculable and efficient method during the cataract surgery of patients who wish to achieve spectacle independence. In Group A, superior uncorrected intermediate vision and better depth of focus was found three months after the surgery, compared to the patients in Group B.
