Abstract Details

Purpose:

To present a new method to correct irregular astigmatism of different causes with improvement of corneal regularity, high order aberrations and refraction

Setting:

78 eyes with Irregular Astigmatism were treated with High Resolution Wave Front guided treatment and simultaneous accelerated Cross Linking. Results are presented here after a mean follow up of 16 months.

Methods:

High Resolution Wave Front guided Excimer Laser Surface Ablation with the AMO Visx followed inmediately for accelerated Cross Linking were performed in 78 eyes with Irregular Astigmatism resulted from Keratoconus (45), Post Lasik ectasia (12 eyes), Radial Keratotomy (16 eyes) and 5 post PK. At 16 months follow up resuktas will be presented in the form of UCVA, BCVA, HOA number, Coma and Spherical aberration as well as manifest refraction.

Results:

All cases improved in UCVA with 3 eyes losing 1 line of BCVA, 45 eyes no change, 22 eyes improve of 1 line of BCVA and 8 eyes improved 2 or more lines of BCVA. HOA decreased as well as Coma and Spherical aberration.

Conclusions:

Excimer Laser surface ablation with High resolution Wave Front guided treatment followed by Accelerated Cross Linking is safe and effective to correct Irregular Astigmatism. Results are better than reported results by Topography Guided Excimer Laser Ablation since with this treatment the irregular shape of the cornea is corrected at the same time as the refractive treatment is performed, as a big difference with Topo guided treatment that only corrects the shape of the cornea and can end up with an unexpected refractive defect.
