Abstract Details

Purpose:

To study the outcome of cataract surgery with IOL implantation in children with buphthalmos.

Setting:

Tertiary Eye Care Centre Advanced Eye Centre, India

Methods:

Children less than 12 years with buphthalmos and cataract who underwent phacoaspiration through anterior (limbal) route were studied. The surgical procedure consisted of phacoaspiration with small (4.5-5mm) anterior capsulorhexis with /without primary posterior capsulotomy (PPC) and primary IOL implantation. IOL implanted was hydrophobic acrylic three piece (Sensar Ar 40,Abott technologies, Santa Ana ,California).

Results:

The mean age at which the children underwent cataract extraction was 6.85 + 2.7 years (range 3-11 years). After making side port incisions and staining the capsule, anterior capsulorrhexis of 4.5-5 mm was done. PPC with anterior vitrectomy was done in 7 eyes. Endocapsular implantation was performed in 4 eyes and rhexis fixation (optic in bag and haptic in sulcus) in ten. Eight eyes developed posterior capsular opacification. IOL remained stable in all eyes with posterior capsulotomy where rhexis fixation was done.Decentration was observed in 2 eyes where IOL was implanted in bag.

Conclusions:

Primary posterior capsulotomy with rehexis fixation of standard 3 piece hydrophobic IOL gives a good surgical outcome in pediatric buphthalmic eyes undergoing cataract surgery.
