Abstract Details

Purpose:

To analyze the causes of unsuccessful phacoemulsification surgery with the use of capsule hooks, which had to be converted to extracapsular cataract extraction (ECCE) in patients with severe subluxations of crystalline lens.

Setting:

S. Fyodorov Eye Microsurgery Federal State Institution. Saint-Petersburg branch. Russia.

Methods:

This retrospective study included 17 patients with severe subluxation of crystalline lens, who were scheduled to have microcoaxial phacoemulsification with IOL implantation in capsular bag. In all cases, capsule support hooks were used.

Results:

Sutureless phacoemulsification with implantation of IOL was successful in 15 out of 17 patients. Ten patients had early nuclear cataracts with soft or medium-hard nucleus. One of them had severe fibrotic changes of the anterior capsule, and during the attempt to enlarge the capsular rhexis, a rupture of the anterior capsule had occurred, which led to the necessity to switch to the ECCE. Five patients had hard brown cataracts. One of them had no vitreous gel due to the history of pars plana vitrectomy and the phacoemulsification was converted to ECCE due to the high risk of dislocation of the nucleus into the vitreous cavity. The remaining two patients presented with hypermature (Morgagnian) cataracts and underwent successful sutureless microcoaxial phacoemulsification.

Conclusions:

System of capsule support hooks significantly increases the chances of successful sutureless phacoemulsification. However, in some extremely complicated cases (fibrotic capsule changes, vitrectomised eye) use of capsular hooks was insufficient to perform sutureless microcoaxial phaco.
