Abstract Details

Purpose:

To evaluate morphological features of clear corneal incisions (CCI) after implantation of a foldable hydrophobic acrylic intraocular lens with 3 devices: manual injector, AutoSert TM motorized injector and the UltraSertTM pre-loaded delivery system, using optical coherence tomography (OCT).

Oftaprof Clinic, Iasi, Romania

Methods:

This prospective non-randomized clinical trial included 90 eyes(90 patients) that underwent standard phacoemulsification using a 2.2mm CCI. The study comprised three groups depending on IOL injector used: Group I(n=30), where where a plunger type injector (Royale) was used; Group II(n=30), where an automated motorized injector (Autosert) was used; Group III(n=30), where the pre-loaded delivery system UltraSert was used. Architectural features of CCIs were evaluated using anterior segment OCT on postoperative day 1.The length of the corneal incision and the angle of the incision were measured. The presence of localDescemet membrane detachment,endothelial misalignment, epithelial and endothelial gaping were assesed.

Results:

The mean CCI length was 1.64 mm +/- 0.22 in Group I,1.65 mm +/- 0.27 in Group II and 1.71 mm +/- 0.2 in Group III. The mean incision angle was 40.9 +/- 8.63 degrees in Group I,41.7 +/- 9.03 degrees in Group II, 41.5 +/- 8.94 degrees in Group III.The difference between groups was not statistically significant (P>0.05) for any parameter.The incidence of epithelial gaping and endothelial misalignment was similar for all groups( P>0.05).The incidence of Descemet membrane detachment was 66% in Group I, 46% in Group II and 40% in Group III. The incidence of endothelial gaping was greatest in Group I followed by Group II and Group III (46%, 40% and 33%, respectively).The difference was statistically significant.

Conclusions:

The motorized insertion system and the UltraSertTM pre-loaded delivery system preserved the architecture of the original corneal incision compared to the manual injector,causing better posterior wound integrity.
