Long-term outcomes of leucosapphire drainage implantation for surgical treatment of refractory glaucoma

Abstract Details

Purpose:

To study long-term clinical results of refractory glaucoma surgery by using leucosapphire drainage system.

Setting:

: St.Petersburg Branch of S.N. Fyodorov IR&TC “Eye Microsurgery”, Russia.

Methods:

The device is made of leucosapphire as a pipe 2,6 mm length with inner diameter of 100 µm. Drainage implantation was done to 128 eyes of 128 patients at the age of 33-82 years (female – 72, male – 56). 14 eyes had II glaucoma stage, 114 – III glaucoma stage. Implantation has been performed after previously done unsuccessful operations. Trabeculectomy was performed for the similar control group. The mean intraocular pressure (IOP) before operation was 33,71±2,22 mm Hg in the basic group and 33,31±2,48 mm Hg in the control group. The mean follow-up was 3,8±1,2 years.

Results:

Intraoperative complications have not been observed. During postoperative period hyperfiltration has been registered in 4 cases (3,1%), choroidal detachment – in 6 (4,7%), hyphema – in 2 (1,6%). The most of these complications have been eliminated therapeutically. Satisfactory IOP control was obtained in all the eyes in a month after operation. However 12-36 month later IOP was normal in 83% of the eyes in the basic group and in 68% of the eyes in the control group.

Conclusions:

Leucosapphire drainage implantation can be chosen from the other methods for the refractory glaucoma treatment. After implantation the postoperative period lasts unreactively because of absence of fibroblastic reaction in sclera to leucosapphire. According to our opinion satisfactory IOP control and long-term drainage function is achieved by unreactive material of the drainage.
