Abstract Details

Purpose:

Mild astigmatism (1 D to 1.25 D) can be corrected by the Toric IOLs, Verion Assisted FLACS Corneal Arcuate Incisions or both. Our purpose was to compare the visual outcomes of Toric IOLs Vs Verion Assisted FLACS Corneal Arcuate Incisions for management of coexisting cataract and corneal astigmatism of 1 D to 1.25 D

Setting:

Retrospective and comparative case series of 72 eyes which underwent Non-Randomized Cataract Surgery by Verion Guided FLACS with Toric IOL Implantation (Group 1, 36 eyes) and Verion Guided FLACS Arcuate Incision (Group 2, 36 eyes) between January 2015 to October 2015 at a Tertiary Care Eye Hosptial, India

Methods:

Retrospective review assessed outcomes of FLACS with aspheric Monofocal toric IOLs and Verion Assisted FLACS Arcuate Incisions with aspheric monofocal IOL for visually significant cataract (NS Grade 3) and coexisting corneal astigmatism of -1 D to -1.25 D under single operating surgeon. Planning of Arcuate Incisions, planning and implantation of Toric IOLs were done with reference of Verion Image System. Visual Recording and Aberrometry (I Trace) were done at 7th and 90th days post-operatively. The main outcome measures were 7 days and 90 days post-operative UCVA, SE, Contrast Sensitivity expressed in Modular Transfer Function and percentage of need of spectacle

Results:

Group 2 yielded better 7 days post operative UCVA (p=0.722), Spherical Equivalent (p=0.270) and Contrast Sensitivity (p=0.614 at 5cpd, p=0.709 at 10cpd) results than Group 1 but were not statistically significant. 90 days post operative results of UCVA (p=0.027), Spherical Equivalent (p=0.019) and Contrast Sensitivity (p=0.041 at 5cpd, p=0.038 at 10cpd) in Group 2 were found to be significantly better than group 1 Percentage of eyes in need of spectacles (47.2%) were same in 7 days but were higher in the Lensx group (63.9%) as compared to Toric IOL group (55.5%) at 90 days follow up.

Conclusions:

Both Toric IOLs and LenSx arcuate incision reduced astigmatism; but the astigmatism correction was less effective in due course of time in the Arcuate Incision as compared to the Toric IOLs. Toric IOL group achieved significantly better post operative Uncorrected Visual Acuity, Contrast Sensitivity and Spherical Equivalent as compared to the Arcuate group resulting in significantly less number of eyes using spectacles post operatively. Toric IOLs reduced astigmatism to a higher extent and effect of toric IOLs were more predictable and sustained.
