Abstract Details

Purpose:

The purpose of this report is to calculate the total energy delivered in the patient’s eye during femtosecond laser preparation for cataract surgery. The calculations pertain to the settings used in clinical practice for two different laser platforms and are based on parameters as provided by the systems.

Setting:

Athens Eye Hospital, Athens, Greece

Methods:

Theoretical calculations based on laser settings and programmed treatment patterns were performed for LenSx (Alcon, Fort Worth, TX) και Catalys (AMO, Santa Ana, CA).

Results:

For a 5mm in diameter capsulotomy the total energies were 0.75J and 1.15J for LenSx and Catalys respectively. For quadrant segmentation (chopping) the energies were 1.63 J and 0.67J LenSx and Catalys respectively. Indicatively, if 1 J of energy is delivered to a volume of water equal to 170μL (similar to a typical anterior chamber) the corresponding temperature rise is approximately 1.4 degrees. It is reasonable to expect that local temperature rise in the crystalline lens during segmentation is higher in comparison to that during capsulotomy as there is no heat transfer by convection.

Conclusions:

The total energy is proportional to the area of the incisions performed, to the pulse energy and inversely proportional to pulse spacing (transverse and axial). The total energy can be significantly varied if these parameters are adjusted. Careful adjustment of these parameters can significantly reduce the delivered energy without substantial difference in the effectiveness of the treatment.
