Abstract Details

Purpose:

In this study we aim to study the complication rate and surgical learning curve of femtosecond laser lens surgeries (FLS) using LenSx® in 1000 consecutive cases in our department.

Setting:

Department of Ophthalmology at the Goethe-University Hospital in Frankfurt, Germany.

Methods:

1000 consecutive cases were analyzed retrospectively at our department. The FLS were carried out using the LenSx® Laser. Phacoemulsification was performed using Infiniti Vision System Unit for the first 630 cases and then switched to Alcon Centurion® Vision System for the remaining 370 cases. The surgical technique was standardized in all eyes and performed by the same surgeon (T.K). Intraoperative complications that occurred were recorded in the data collection Excel sheet. A learning curve was analyzed by comparing the first 200 FLS cases (group 1) with the following 800 consecutive cases (group 2) by a Chi² test.

Results:

In the first 200 FLS cases, a total of 35 occurrences were seen (17.5%) which included manual incisions, suction problems and one radial tear. Whereby, only 66 eyes in the following 800 cases had intraoperative incidents (8.3%, p < 0.01). Manual incisions were performed in 30 cases in group 1 and 46 in group 2 (p <0.01). 3 surgeon-related posterior capsular ruptures were noted in group 2. The following incidences were also observed in the 800 eyes: one capsular tag, one capsulorrhexis enlargement and two capsular sack tears during irrigation-aspiration that required anterior vitrectomy followed by in-the-bag intraocular lens implantation.

Conclusions:

LenSx® Laser has shown to be a safe and effective means of performing FLS. The rate of complications were found to have significantly reduced which could be attributed to improved surgeon experience in addition to optimizations made to the laser platform.
