Abstract Details

Purpose:

To evaluate the outcomes of Supra-Tenon Excision of Pterygium (STEP) surgery with conjuncival autograft in eyes with primary pterygium . It was evaluated on the basis of ease and time duration of surgery, post operative cosmetic outcome, complications and recurrence.

Setting:

Disha Eye Hospitals, Barrackpore, Kolkata, India. It was a retrospective, non comparative case study where total 14 eyes with primary pterygium were included. Any eyes wth prior intra-ocular surgeries were excluded.

Methods:

It is speculated that excessive proliferation of Tenon’s capsule leads to recurrence after pterygium surgery. So if anyone excise pterygium without disturbing Tenon’s capsule the chance of recurrence can be reduced or abolished. With this idea, pterygium head is excised at limbus and body was excised without disturbing Tenon’s capsule. The scar tissue over cornea below pterygium head was smoothened by crescent knife.The raw area of Tenon’s capsule was covered by Tenon free superior bulbar conjunctival autograft without using suture or glue. The eye was patched overnight.

Results:

There were 10 males and 4 females. Age ranged from 26 to 51 yrs with a mean of 36.3 yrs. Follow up ranged from 90 to 130 days with a mean of 109.6 days. Eyes with follow up less than 90 days were excluded. Surgical time duration ranged from 7 to 12 minutes with a mean of 8.5 minutes. It was faster than conventional pterygium excision with conjunctival autograft. All patients with STEP had better cosmetic outcome as episcleral blood vessels were not disturbed or cauterised. Even though, the follow up period is short, not even minor complications or recurrence found in post-operative period.

Conclusions:

The STEP surgery is easier, much faster and safer way of excising pterygium. All patients with STEP had amazing cosmetic outcome as episcleral blood vessels were not disturbed or cauterised. As it is a suture- less and glue free surgery the surgical time is quite less and no unwanted side effects of either glue or suture and the eye becomes quiet in a week of time.
