Intracorneal ring segment implantation in young patients under 20 years old with keratoconus: clinical results and disease progression after a 2 year follow-up

Purpose:
To analyze visual, refractive and topographic results after Ferrara-type intracorneal ring segments implantation (ICRS) for keratoconus treatment in patients under 20 years of age.
Setting:
Department of Ophthalmology,Hospital de Braga, Portugal
Methods:
The medical records of 34 eyes (28 patients) with Keratoconus grade 1-3 (Amsler) submitted to implantation of ICRS (manual technique) who were <20 years old at the time of the procedure were reviewed. The following parameters were analyzed: uncorrected distance visual acuity (UDVC), corrected visual acuity (CDVA), refractive sphere and cylinder, keratometry (k), corneal aberrometry and central corneal thickness (CCT). Patients were evaluated at 6, 12 and 24 months.
Results:
The mean UDVA improved from 0,17 ± 0,15 to 0,34 ± 0.21 (p<0,05) (decimal scale) and CDVA improved from 0,47 ± 0,22 to 0,62 ± 0,14 (p<0,05) at 24 months follow-up. Mean sphere and cylinder improved from -1,45 ± 2,45 D to -0,50 ± 1,70 D (p=0,07) and from 3,46 ± 2.24 D to 2,06 ± 1.46 D (p=0,0056) at 24 months, respectively. At 6 months, mean K1, K2 and Kmax values decreased (p<0,05). Visual, refractive and topographic parameters were stable between 6, 12 and 24 months, with no statistically significant differences during the follow-up after the immediate postoperative result.
Conclusions:
The implantation of ICRS showed good visual, refractive and topographic results in patients under 20 years of age while maintaining stability along a 24 months follow-up period, showing its potential to halt the disease progression even in a young patient population.
