Abstract Details
Purpose:
To demonstrate technique and intermediate results of intrascleral fixation of hydrophobic acrylic one-piece posterior chamber intraocular lens (PCIOL) in eyes with no or insufficient capsular support.
Setting:
Aurelios Augenzentrum, Recklinghausen, Germany
Methods:
After opening the conjunctiva two straight ciliary sulcus sclerotomies are prepared with a 22-gauge cannula 2.0 mm postlimbal, 180° apart from each other. A small intrascleral pocket is prepeared with a sharp needle counterclockwise, parallel to the limbus, starting from the sclerotomies. The haptics of a hydrophobic acrylic one-piece IOL (AcrySof®, Alcon Lab, USA) are fixated intrasclerally with the help of a special 25G forceps (Scharioth Forceps, DORC, The Netherlands). Surgical technique will be demonstrated with video. Refraction, intraocular pressure, PCIOL position and complications were evaluated.
Results:
All PCIOL with intrascleral haptic fixation were stable and well centered. No intra- or postoperative complications occurred in a postoperative follow-up of maximum 36 months. Intraoperative orientation of the haptic during intrascleral fixation seemed to be more difficult as using three-piece PCIOL for this technique.
Conclusions:
Our results of sutureless intrascleral fixation of one-piece PCIOLs suggest that it is a reliable and successful method with low complication rate, resulting in long-term centration, three-dimensional axial stability for reliable refractive results in eyes with no or insufficient capsular support.
