Abstract Details

Purpose:

To compare postoperative macular thickness changes after posterior fixated iris-claw lens implantation and scleral fixated intraocular lens (IOL) implantation for aphakia rehabilitation.

Setting:

Balikesir University, Medical Faculty, Ophthalmology Department

Methods:

Patients who underwent secondary IOL implantation surgery to treat aphakia secondary to cataract surgery were included. Eyes in Group 1 underwent posterior fixated iris-claw lens implantation and eyes in Group 2 underwent scleral fixated IOL implantation. Central, inner and outer zone macular thicknesses were analyzed with spectral domain optical coherence tomography preoperatively and postoperatively at 1st, 3rd, 6th and 12th months. Duration of the aphakia was also recorded. Two groups were compared regarding macular thickness changes.

Results:

Forty nine eyes of 49 patients were included. Group 1 consisted of 16 patients with a mean age of 68.29±6.9 years and Group 2 consisted of 33 patients with a mean age of 68.8±14.5 years (p=.40). The mean central foveal thickness was 236.5±33.3 microns in Group 1 and 240.9±43.5 microns in Group 2 (p=.95). Macular thicknesses in all zones were significantly increased at months 1, 3 and 6 in both groups (p<.05). At 12th months macular thickness decreased to preoperative values in almost all cases. No significant difference was observed between the groups in terms of macular thickness changes at any postoperative visit (p>.05). The aphakia duration had no influence on macular thickness in both groups (p>.05).

Conclusions:

The current study demonstrated that macular thickening may occur after secondary IOL implantation surgery during the postoperative 6-months. However, macular thickness returned to preoperative values at 12th month. Both techniques; posterior fixated iris-claw lens and scleral fixated IOL implantation have similar effects on macular thickness.
