Purpose:
To assess the efficacy, safety and predictability of the sequential implant of intracorneal ring segments (ICRS) and monofocal intraocular lens (IOL) in patients with keratoconus and cataract.
Setting:
Instituto Universitario Fernández-Vega (Oviedo). Spain.
Methods:
Prospective study including patients diagnosed of keratoconus and cataract who underwent an ICRS implant, following Mediphakos (Brasil) nomogram and six months later a monofocal IOL. Lens calculation was performed by interferometry, using the SRK-T formula. Uncorrected and corrected distance visual acuities (UDVA & DCVA respectively) and residual refraction were analysed by vector analysis. The parameters object of this study were recorded preoperatively, six months after the ICRS implant and six months after the IOL implant.
Results:
The study included 70 eyes from 47 patients. Mean preoperative UDVA was 1.08 ± 0.25 and 0.95 ± 0.31 (logMAR scale) at six months from the ICRS implant (p=0.03). At six months after the IOL implant it was 0.44 ± 0.29 (p<0.0001). Mean DCVA changed from 0.35 ± 0.23 before surgery to 0.28 ± 0.22 at six months after the ICRS implant (p<0.0001) and to 0.11 ± 0.16 at six months after the IOL implant (p<0.0001). The efficacy index was 0.87 and safety index 1.61, both calculated at the sixth month after IOL implant.
Conclusions:
The sequential implant of ICRS and monofocal IOL provides good visual and refractive results, indicating that it is a predictable procedure for patients with keratoconus and cataract.
