Abstract Details
Purpose:
The aim of this study is to compare and evaluate new preloaded, intraocular lens (IOL) injector systems for IOL implantation in porcine eyes.

Brighton & Sussex University Hospitals NHS Trust, Brighton, UK
Methods:
The preloaded lenses analysed in this study of 25 porcine eyes were: UltraSert®, EyeCee®, iSert®, RayOne® and CTlucia®. Five IOLs, 22D each, were implanted for each company. The same surgeon performed all surgeries. Data on pre and post IOL implantation wound size was collected. Postoperative internal wound size was measured from inside, after opening the eyeball through a sclerostomy. Using Likhert scale, we evaluated the ease of: opening the packaging, viscoelastic priming, IOL advancement into the nozzle, nozzle insertion into the wound and IOL delivery. Likhert scale was from 1-4 for each step: 1=very difficult, 2=difficult, 3=easy, 4=very easy.
Results:
Pre-implantation external incision was 2.2mm in all. Only 2 (Ultrasert and RayOne) scored 4.0/4.0 for all aspects of ‘ease of use’. Ultrasert and EyeCee took half the time to operate when compared with iSert and CTlucia, and 50% less time than RayOne. Postoperative incision size (mean size, wound stretch) was smallest in RayOne (2.48±0.04mm, 12.7%) compared to Ultrasert (2.54±0.05mm, 15.5%), EyeCee (2.60±0.06mm, 18.2%), iSert (2.60±0.0mm, 18.2%) and CTlucia (2.64±0.05mm, 20%). 1 EyeCee (20%) and 2 CTLucias (40%) failed to insert and remained stuck in the nozzle/incision. RayOne, Ultrasert and CTLucia showed significantly less nozzle damage post-injection than iSert and EyeCee.
Conclusions:
Different companies have different preloaded IOL injection systems with some variations amongst them. Ultrasert and RayOne are easier to use. Ultrasert and EyeCee were faster to operate. All systems give postoperative wound stretching with RayOne being the least and Ct lucia being the maximum. Ultrasert, RayOne and CT lucia showed less nozzle damage post injection.
