Purpose:
To evaluate the long term efficacy and safety of a novel technique of releasable suture used in combined phacoemulsification (Phaco) with acrylic intraocular lens implantation and Non-Penetrating Deep Sclerectomy (NPDS).
Setting:
Private practice. Instituto de Oftalmologia Tadeu Cvintal, Sao Paulo, Brazil.
Methods:
A retrospective chart analysis was performed on 33 eyes of 27 patients after a combined Phaco/NPDS procedure performed by one surgeon. Followed up for an average of 1 year for best corrected visual acuity (BCVA), intraocular pressure (IOP), number of glaucoma medications (NGM), time of removal of the sutures, IOP at the time of the removal of the sutures, short and long term complications.
Results:
Mean preoperative variables were: BCVA, 0,43 (SD +- 0,31) logMAR; IOP, 18,07 (SD +- 6,13) mmHg; and NGM, 1,6 (SD +- 0,81). Mean postoperative results: BCVA, 0,17 (SD +- 0,58) logMAR; IOP 14.15 (SD +- 0,73) mmHg, NGM 0,18 (SD +- 0.73). At one year, 94% of the eyes had no glaucoma medication. The first releasable suture was removed after 5,3 (SD +- 3,7) days from the surgery at a mean IOP of 14,05 (SD +- 0,73) mmHg. The second releasable suture was removed after 9.9 days from the first suture at a mean IOP of 14,6 (SD +- 7,0) mmHg. During the early post-operative period, 2 eyes presented with Seidel and another two eyes with shallow anterior chamber.
Conclusions:
This novel releasable suture technique for combined phacoemulsification and deep sclerectomy surgery is a straight forward, safe, and effective alternative for patients with cataracts and glaucoma.
