Purpose:
Implantation of presbyopia correcting intraocular lenses (IOL) after laser refractive surgery is still controversial. Lens power calculation, visual disturbances caused by irregular corneal shape and dryness of the eye are possible reasons for patient’s dissatisfaction after cataract surgery. We present our experience with implantation of Tecnis Symfony IOL in patients who had previous laser refractive surgery.

University Eye Hospital Svjetlost, Zagreb, Croatia
Methods:
Cataract surgery after LASIK was performed to six patients, twelve eyes with implantation of Tecnis Symfony IOLs. All patients were myopes prior to LASIK. We targeted emmetropy in dominant eye and mild myopia, up to -0,50 D in non dominant eye. Aramberri “Double K” method correction was used for IOL power calculation. Uncorrected distant visual acuity (UCDVA), uncorrected intermediate visual acuity (UCIVA) at 66 cm, uncorrected near visual acuity (UCNVA), spectacle independence, patient satisfaction and visual disturbances were measured. Follow up was at least 6 months.
Results:
Cataract surgery was performed 3 to 7 years after LASIK. Mean preoperative best corrected distance visual acuity (BCDVA) was 0,51±0,20. Mean monocular post cataract UCDVA was 0,92±0,10, mean UCIVA at 66 cm was 0,87±0,10 and mean UCNVA was 0,87±0,15. Mean subjective postoperative residual dioptry was 0,33±0,32 D. Two patients sometimes wear glasses for reading small print. All patients reported transitory dryness of the eye. Four of six patients reported mild haloes. Patients were highly satisfied, without severe complaints.
Conclusions:
In this group of myopic post LASIK cataract patients who underwent cataract surgery with implantation of Tecnis Symfony IOL we achieved satisfying visual acuity, high spectacle independence and low visual disturbances such as halos and glare. Precise IOL power calculation and careful patient selection concerning post LASIK corneal topography and ocular surface status are crucial for success.
