Capsulaser capsulotomy: clinical experience with the first 20 cases with FineVision trifocal IOL

Abstract Details

Purpose:

Introduction of the new laser capsulotomy method which does not require docking.

Setting:

Gemini Eye Clinic, Zlin, Czech Republic

Methods:

Capsulaser (EXCEL-LENS, USA) was used to create laser capsulotomy in 20 eyes with cataract without any other intraocular pathology or any previous surgery. After incisions and intraocular anesthesia a trypan blue solution was applied to anterior chamber on anterior lens capsule and was washed out by Ringer solution after 30 seconds. The surgical contact lens was placed on the cornea. Then the circular capsulotomy of 5,0mm diameter was created by CapsuLaser. After phacoemulsification a 13mm capsular tension ring and trifocal FineVision (Physiol, Belgium) were implanted. Distance and near UVA and CVA were assessed 1 week and 1 month postoperatively.

Results:

Laser capsulotomy by CapsuLaser (EXCEL-LENS) took 1 second in all cases. Continuous well-centered circular capsulotomy was achieved in all 20 eyes. Phacoemulsification was uneventful in all cases. Trifocal IOL FineVision (Physiol) implantation was successful in all cases. No radial anterior capsular tear occurred during surgery. Mean UDVA was 0,9 and mean CDVA was 1,0. Mean UNVA and CNVA were J1. All lenses appeared well centered during the follow up period. No capsular fibrosis was noted.

Conclusions:

The first clinical experience with laser capsulotomy by CapsuLaser (EXCEL-LENS) device is very positive. The capsulotomy is easy to perform, the time of surgery is shorter than femtosecond laser capsulotomy with no need for docking. The laboratory measurements showed higher strength and tear resistance of CapsuLaser capsulotomy compared to manual capsulorhexis. CapsuLaser device is less expensive than femtosecond laser platforms with significantly lower per case cost. CapsuLaser technology can make laser capsulotomy more affordable and significantly easier available for cataract surgeons worldwide compared to current technology.
