Abstract Details

Purpose:

To investigate different capsultomy centration methods in image-guided laser-assisted cataract surgery.

Setting:

University Eye Hospital Bochum, Germany

Methods:

In this prospective randomized trial the intraocular lens (IOL) -capsule overlap was compared between two different capsulotomy centration methods. In the first group (three-piece IOL) and second group (plate haptic IOL) the anterior capsulotomy was centered on the pupil. In the third group (three-piece IOL) and fourth group (plate haptic IOL) the capsular bag was reconstructed by 3D spectral domain optical coherence tomography and the midpoint of the bag was used for centration of the capsulotomy (scanned capsule method).

Results:

In total 160 patients were included. There was no difference between the groups regarding age and cataract grade. With a 5.0 mm capsulotomy a 360 degree overlap of the IOL was achieved in all patients with the scanned capsule method (group three and four). Also in group two (pupil centration – plate haptic IOL) a complete overlap was achieved in all patients. In group one (pupil centration – three-piece IOL) three patients (7.5%) did not have a complete overlap.

Conclusions:

The capsular bag centration is a new and promising method in laser-assisted cataract surgery. Especially patient with an irregular pupil dilatation or circumscribed zonular defects might profit from this approach.
