Abstract Details

Purpose:

To investigate the clinical efficacy of 10% phaco system (almost zero phaco) and also evaluate the YAG capsulotomy rates due to posterior capsule opacification (PCO) following femtosecond laser assisted cataract surgery (FLACS) in Japanese eyes.

Setting:

Nishi Eye Hospital, Osaka, Japan

Methods:

160 eyes underwent cataract surgery. CCC and lens fragmentation were completed by femtosecond laser (Catalys, AMO). 10% phaco system (MST/Packard 0.7mm bent phaco tip, Dewey Reverse Bavel&gold silicone sleeves) was applied for the last 25 eyes. The average effective phaco time (EPT) and US time were calculated. The change of corneal thickness and postoperative inflammation were also investigated. Then, the data was compared with that of the matched conventional group. Among all, 30 eyes were implanted with Technis Z9002 IOLs (AMO). The postoperative YAG rates of this sharp edged 3-piece silicone IOL in FLACS were evaluated as preliminary investigation.

Results:

The average EPT of the 10% phaco FLACS and conventional group was 15.6+/-44.8 sec.and 45.0+/-78.0 sec. each (p<0.05). The average US time in 10% phaco system was 80+/-94 sec. There was a significant difference regarding the change of corneal thickness. (10+/-14% and 22+/-16% each at Day1, p<0.05) There was no significant difference regarding the postoperative inflammation between the two groups (15.6+/-9.6 pc/ms and 24+/-11.2 pc/ms each at Day1). YAG rates of Technis Z9002 IOLs after FLACS were 11 % (3 eyes / 28 eyes) during the 1 year postoperative follow-up. 3 years following FLACS, the YAG rates were 20% (4 eyes/20 eyes).

Conclusions:

10% phaco system in FLACS was found to be very efficient, reducing the phaco energy and time for removing cataracts safely. This system may be also useful in removing hard cataracts smoothly in FLACS by using minimum phaco energy. Postoperative cytokines might have affected PCO formation after FLACS. Effective conditions and settings in FLACS should be further investigated. More clinical reports about long-term PCO and YAG capsulotomy incidence in various intraocular lenses following FLACS are definitely required.
