Purpose:
To present the early experience in Lasik flap creation with a multifunctional femtosecond laser for cataract, refractive and corneal procedures.
Setting:

Methods:
The first fifty eyes that underwent LASIK flap creation with a multifunctional femtosecond laser (LenSx, Alcon Laboratories, Inc., Fort Worth, TX)) for correction of refractive errors were analyzed. The frequency of the Femtolaser was switched from 50 to 150 kHz, in order to create corneal flaps. The diameter of the flap and the flap thickness, was 9.0mm and 110 or 120μ, respectively, in all cases. Procedure duration, flap bed and side cut parameters were documented.
Results:
Switching time between cataract and refractive mode was 8 minutes. The initial spot bed energy of 1μJ was increased to 1.2 μJ after the first cases. Tang spot separation was decreased from 7 to 6μ and radial spot separation was increased from 7 to 8 μ. Additionally, side cut spot energy was increased from 1.5 to 2.05 μJ and spot and layer separation decreased from 5μ, to 3μ and 2μ, respectively. The average duration of the procedure, docking to undocking and laser emission time, was 92 sec and 16 sec, respectively. No flap-related complications, but one, occurred in the first fifty cases. An incomplete flap separation occurred in the second eye of a patient due to severe eye movement.
Conclusions:
The LenSx femtolaser was able to create LASIK corneal flaps effectively and safely. Spot energy and separation adjustments for flap bed and side cut, other than initially recommended by the manufacturer, were necessary to produce easily separable flaps.
