Purpose:
To establish the association between aberrometry and geometric and morphometric characteristics in keratoconus disease.

Methods:
This was a retrospective case series study evaluating a sample (34.72 ± 11.12 years) divided into five groups according to an aberrometric grading scheme: 102 healthy eyes, 10 grade I, 9 grade II, 4 grade III and 6 grade IV. Morphogeometric characteristics were established using an in vivo, non finite global 3D analysis of the cornea. Data engagement scores were confirmed by means of the Kolmogorov–Smirnov test. According to this analysis, a Kruskal-Wallis test was used to compare differences between groups according to disease stages (Alió-Shabayek grading system). Pairwise comparisons were performed using Dunn's procedure with a Bonferroni correction.
Results:
There were differences between groups in the following variables: Anterior corneal surface area (p<0.000), Posterior corneal surface area (p<0.000), Sagittal plane apex area (p<0.000), Sagittal Plane Area in minimum thickness points (p<0.000), Anterior apex deviation (p<0.000), Posterior apex deviation (p<0.000), Anterior minimum thickness point deviation (p<0.008), Posterior minimun thickness point deviation (p<0.011).
Conclusions:
Due to corneal irregularity in keratoconus disease, an increase of optic aberrations could be observed according to the disease progression. Morphogeometric analysis allowed to differenciate between groups according to the Alió-Shabayek aberrometric grading scheme.
