Purpose:
To find the risk factors for extension of the capsulorhexis in intumescent cataracts by intraoperative real time imaging using the RESCAN™ 700 (Carl Zeiss Meditec, Oberkochen, Germany)
Setting:
Department of Cataract and Refractive Surgery, Narayana Nethralaya, Bangalore
Methods:
Thirty eyes of thirty patients with intumescent cataracts were imaged on the RESCAN™ 700 Microscope integrated with Intra-Operative Spectral Domain Optical Coherence Tomography (SD-OCT). The continuous curvilinear capsulorhexis was performed under Viscoat with micro-rhexis forceps, after an attempted careful decompression of the lens. These images were later analyzed to find the risk factors for a capsulorhexis extension. Factors like curvature of the anterior capsule, elevation of the lens center from the periphery and lens morphology were assessed.
Results:
Out of the 30 eyes, 5 eyes showed capsulorhexis extensions despite taking all the standard precautions. 3 out of the 5 eyes showed a well hydrated cortex without large fluid filled cavities. This meant that capsulorhexis run off could be predicted and prevented with the necessary decompression techniques. Lens morphology and curvature of the anterior capsule seemed to be better indicators in predicting a run-off as compared to the rest.
Conclusions:
Intra-operative SD-OCT integrated with the operating microscope is an interesting new technology which would help us in real time in-vivo assessment and understanding of the hurdles faced during cataract surgery. It has a very promising role in decision making during the surgery and training.
