Abstract Details

Purpose:

Mitomycin-C (MMC) is used to prevent postoperative corneal haze formation after different modalities of photorefractive keratectomy. The threshold ablation depth to indicate MMC application is controversial. We aimed to compare efficacy, stability and safety of single-step transepithelial photorefractive keratectomy (TransPRK) with or without mitomycin-C (MMC) in eyes with ablation depth of 160 µm or below.

Bina Eye Hospital, Tehran, Iran

Methods:

In this comparative case series study, 40 patients with myopia (up to -5.5D) with or without astigmatism that underwent single-step TransPRK by the same surgeon (S.A.M.) using SCHWIND AMARIS 500 laser were included. Total laser ablation depth (epithelium and stroma) in each eye was 160µm or lower. After the ablation, sponge soaked with 0.02% MMC was applied on right eye of the patient for 5-14 seconds. Left eye did not receive any MMC. All the eyes received Iran regimen as postoperative medication. Data of optical assessments before, and 1, 3, 6, and 12 months after the operation were collected.

Results:

Preoperative refraction, visual acuity, CSs and HOAs of right and left eyes were comparable. Postoperatively, we detected one right and two left eyes with 2+ grade of haze (P>0.99), and two other left and two other right eyes with 1+ degree of haze (P>0.99). No eye suffered from induction of astigmatism. All of the eyes achieved stable refraction by third month of follow-up. 38 (95.0%) right and 39 (97.5%) left eyes achieved UDVA≥20/20 (P > 0.99). Right and left eyes achieved comparable postoperative photopic (P=0.8) and mesopic (P=0.7) CSs, and spherical (P=0.4), cylindrical (P=0.3), coma (P=0.7), and trefoil (P=0.6) HOAs.

Conclusions:

Our findings demonstrated similar efficacy, safety and stability of single-step TransPRK with or without MMC, in correction of myopic eyes with total ablation depths equal or below 160 µm. This questions application of MMC in this subgroup of patients.
