Abstract Details
Purpose:
To evaluate changes of angle kappa after multifocal intraocular lens (IOL) implantation.
Setting:
Department of Ophthalmology, Semmelweis University, Budapest
Methods:
Refractive cataract surgery with multifocal IOL implantation was carried out in 60 eyes of 30 patients. After phacoemulsification Medicontur Bi-Flex 677MY IOL was implanted into 24 eyes of 12 patients and Alcon Acrysof Restor SN6AD1 IOL into 36 eyes of 18 patients.
During preoperative examination optical biometry was carried out using Lenstar LS 900 for IOL calculation and to measure axial length (AL), keratometry, lens thickness, anterior chamber depth and angle kappa (d, x- and y-offset).
3 months after cataract surgery biometry measurements were repeated and postoperative IOL position and also angle kappa was determined using Scheimpflug imaging (Galilei G4).
Results:
Preoperatively significant correlation was found between AL and angle kappa (R=-0.502; p<0.001) and between AL and x-offset (R=0.546 p<0.001).
Angle kappa (p=0.0016) and x-offset (p<0.001) decreased significantly after surgery.
After adjusting for AL and preoperative angle kappa, type of IOLs was not a significant predictor of postoperative angle kappa (p=0.848) in a multivariable general estimating equation model.
There was no difference in angle kappa measured by Lenstar and Galilei postoperatively (p=0.058).
IOL decentration and angle kappa showed significant correlation (R=0.286 p=0.042) postoperatively. Significant correlation was noted between horizontal decentration and x-offset (R=0.321 p=0.0178) and vertical decentration and y-offset (R=0.402 p=0.0042).
Conclusions:
Values of angle kappa decrease significantly after cataract surgery. Type of implanted multifocal IOLs has no influence on postoperative values.
A significant displacement of visual axis is observed after multifocal IOL implantation and relates to the position of the IOL centre measured by Scheimpflug imaging.
