Purpose:
To evaluate the safety and efficacy of Small incision refractive lenticule extraction for treatment of pediatric high myopic anisometropia and contact lens(CL) intolerance in 18 children.

Methods:
The prospective, noncomparative interventional case series comprised 18 eyes of 18 children aged 5 to 12 years with high myopic anisometropia and amblyopia who were unable to use CL and glasses. SMILE was performed with sedation or general anesthesia., surgery was followed by a half-day patching of the dominant eye. Main outocome measures were cycloplegic refraction, , visual acuity (UCVA, BSCVA., topographic stability and binocular vision outcome over one year follow up.
Results:
The mean preop. spherical equivalent (SE) refraction was -9.96 diopters (D) and the mean postop. SE -1.48 D. The mean preop UCVA 0.03 increased to 0.58 postop. The mean preop. BSCVA was 0.56 and changed to 0.72 postop. All the eyes had no line lost in BSCVA, 8 eyes had one line gained, 4 and 6 eyes had two and three lines gained, resp. All eyes showed topographic stability.
Conclusions:
SMILE is effective and safe methods which may reduce high myopic anisometropia, improve visual acuity and binocular vision in children when CL intolerance.
