Effect of capsular tension ring (CTR) implantation on postoperative rotational stability of a toric intraocular lens

Purpose:
To compare the postoperative rotational stability of a toric intraocular lens (IOL) in patients with implanted capsular tension ring (CTR) and patients without CTR.
Setting:
European Eye Clinic Lexum, Prague, Czech Republic.
Methods:
This cohort study included 66 eyes of 42 patients who underwent uneventful cataract surgery with implantation of a toric IOL (enVista toric MX60T, Bausch Lomb Inc.) to correct preoperative corneal astigmatism. Average patient age was 67±10 years (42-89 years).
The dataset was divided into two groups based on the presence or absence of capsular tension ring (Videris, 11 SR).
First group contained 32 eyes with implanted CTR with average axial length (AL) of 24.63±1.51 mm (22.00 to 29.00 mm), while second group contained 34 eyes without CTR with average AL of 23.40±1.25 mm (21.50 to 26.50 mm).
Results:
In eyes with CTR, mean preoperative manifest astigmatism decreased from -1.31±1.24 D (-4.75 to 0.0 D) to postoperative -0.27±0.32 D (-1.00 to 0.00 D).
In group without CTR, astigmatism decreased from -1.72±1.03 D (-4.0 to 0.0 D) to postoperative -0.32±0.31 D (-0.75 to 0.00 D).
Axis alignment was achieved in 84% (CTR) and 79% (no CTR). Mean IOL misalignment was 3.38° in eyes with CTR, 3.94° in eyes without CTR (p=0.994).
In eyes with AL of 24 mm or more, postoperative toric IOL axis matched the planned axis in 90.5% of eyes with CTR and 75.0% of eyes without CTR.
Conclusions:
In the study of 66 eyes after cataract surgery with implantation of toric IOL we did not observe significant difference in the postoperative rotational stability of the lens position with respect to the presence or absence of implanted capsular tension ring.
Nevertheless, in eyes with axial length of 24 mm or more, we observed a closer match with the planned IOL axis in the group of eyes with implanted endocapsular ring compared to the group without the ring.
