Purpose:
To evaluate changes in regional corneal epithelial thickness after accelerated corneal collagen cross-link¬ing (A-CXL) in eyes with keratoconus using spectral-domain optical coher¬ence tomography (SD-OCT).

Methods:
Anterior segment SD-OCT (RTVue-100; Optovue, Inc., Fremont, CA) was used to compare re¬gional corneal epithelial thickness in eyes with keratoconus before A-CXL and 1, 3, and 6 months after A-CXL. The epithelium map was used to measure epithelial thickness at 17 points within a 6mm central diameter.
Results:
Twenty-four eyes from 18 patients were evalu¬ated. After CXL, average epithelial thickness across all points was initially thinner than before A-CXL at 1 month (11.34 ± 4.5 μm), but this trend reversed, with progressive thickening across points at 3 months (.06 ± 3.8 μm) and 6 months (.64 ± 4.1 μm) after CXL.
Conclusions:
Epithelial remodeling oc¬curs in the early postoperative period after accelerated CXL in eyes with keratoconus, with initial overall epithelial thinning followed by progressive epithelial thickening that does not plateau by 6 months after treatment.
